Chapin Pediatrics is pleased to announce that Dr. Jerri Lynn Shealy

will be on staff beginning August 18th, 2008.   

 

Dr. Shealy will be in the office on

Mondays, Thursdays and Fridays from 8:00 am to 1:30 pm and

Wednesdays from 8:00 am to 5:00 pm.  

 

If you would like to have your child’s medical records transferred to

Chapin Pediatrics please complete the Authorization For Release of

Protected Health Information TO Chapin Pediatrics Form

and fax or bring it to our office.

 

Please also remember, as a new patient, to fill out a Patient Information Form.  Please mail or fax that form to us at your soonest convenience.

are honored to offer care to those who follow her.
